A keto diet doesn't necessarily mean a low-sodium diet... That would be the first thing that I would look at. If you haven't already, I would set your sodium targets between 1500-2000 mg per day (the lower end being more important if your blood pressure is high). You may also need to visit your MD for a review of medication as shifts in fluid balance/drop in the intake of potassium/weight changes can all impact how your medication is working.
